New Patient Forms
Before Your First Appointment
Prior to attending your initial appointment, please register at this link https://centerforsharedinsight.secure-client-area.com and follow prompts to create a login. Once you login, please complete forms that are required and any additional forms requested by your therapist.
For adolescent clients and their parents, please complete forms with "adolescent" in the title. Please have adolescents read and sign the forms, as well parents.
If you have questions about forms, we will discuss them when we meet. If you are not able to complete the paperwork prior to attending your first appointment, please notify your therapist in advance and arrive 20-30 minutes early to have adequate time to complete paper forms before your appointment time.
When you arrive in the Denver office, please have a seat in the waiting area until your therapist comes to greet you at your appointment time. Please feel free to help yourself to complimentary water and tea.
Please be aware that there are multiple therapists practicing in this space, so please end all phone or in-person conversations before entering the waiting area and be mindful of how your voice or other sounds easily carry through the space. The suite therapists want to do everything possible to ensure a quiet space for therapy to occur.
Therapy appointments are 45-50 minutes long and scheduled on a weekly basis or as needed. Appointments are generally scheduled for the same day and time each week to establish rapport, promote consistency, and enhance treatment outcomes. Treatment can be short or long-term, depending on your treatment needs. Once you schedule an ongoing appointment, that appointment day and time is reserved for you. Keeping regular appointments will enhance treatment outcomes. Please refer to Center for Shared Insight’s 24-hour cancellation policy.
This appointment (50-55 minutes) will involve an assessment of your therapy goals, relevant background information, and discussion around the therapist-client fit. Your therapist will also discuss initial treatment recommendations, including expected length and frequency of treatment. Initial forms, payment, confidentiality agreements, and other practice policies will be reviewed.
